Fines Follow Death of Oil Worker

Aberdeen sherff curt heard that they were workng n the tank wth colleague chrstopher lnney despte safety procedures allowng for amaxmum of twomen n the confned space. procurator fscal ernest barbour sad aker constructon superntendent samuel mckellar had arranged work tattoo needles, but these were later amended absence, specfyng a maxmum number of two people the tank. he sad the three men had not used rado equpment and had reled nstead on hand sgnals when hostng the materal up to the crew through the small shaft on the deck above. barbour sad it appears that a part of the load that was beng lfted, whch the work party n the tank thought had gone out through the hatch, fell down nsde the tank. the tems that fell were the four angle clamps and shackle. one of the yellow angle clamps appears to have struck matthew grey, the second yellow angle clamp appears to have ht a large valve lyng on the scaffold deck, bounced and ht norman Plastic Parts. amedc rushed to the scene and mr grey was pronounced dead a short tme later. the court heard there was a falure communcaton between the aker superntendent and the talsman area authorty whch ssue permt changes. mrbarbour also sad aker faled to ensure rsk controls were mplemented. talsmans solctor, jack davdson, sad the ol company had taken on a raft of new procedures to ensure the tragedy dd not happen agan. it was not an operaton that should have gone wrong. there were a number of falures on behalf of my clents company, partcularly relaton to the conflct of havng these two actvtes lftng and cuttng takng place at the same tme. akers solctor, rhona jameson, also apologsed for the falures leadng to the Injection Molding. sherff  duglas cusne commended both companes for changng procedures followng the tragedy. the companes were each fned talsman admtted a falure to provde a safe system of work, dentfy the rsks nvolved of lftng and cuttng operatons and ensurng the procedure was properly planned. aker kvaerner admtted falng to provde a safe system of work and falng to ensure the correct number of people allowed to work durng the procedure.
